Get to Know the Collaborative

The International Goal-Oriented Care Learning Collaborative (GOCLC) is a non-profit network that seeks to advance the adoption of Goal-Oriented Care in practice through convening care providers, health care system leaders, and researchers working to transform health systems. 

 

We believe Goal-Oriented Care offers a practical and applied approach to person-centred care delivery. By using personalized goals as the aim of health care encounters, we ensure that care decisions are always aimed at what is most important to individuals and their families. 

 

The GOCLC provides a platform to enable learning across geographic and disciplinary boundaries, as a means to better advance implementation of Goal-Oriented Care. As we grow, we will continue to create these opportunities through: convening and sharing knowledge at relevant conferences and meetings, creating opportunities for knowledge sharing via webinars and newsletters, helping members create new collaborations to advance research and knowledge in the field. 

 

Our Collaborative was founded through our connection to the North American Primary Care Research Group (NAPCRG). We use the NAPCRG Annual meeting held in Fall as an opportunity to connect with many of our members and share new knowledge.

CAROLYN STEELE GRAY
MA,PhD

Dr. Carolyn Steele Gray is a Scientist at the Bridgepoint Collaboratory for Research and Innovation at the Lunenfeld-Tanenbaum Research Institute and an Assistant Professor in the Institute for Health Policy, Management and Evaluation at the University of Toronto, Canada. Her program of work focuses on the role of digital health in supporting integrated and person-centred care delivery for patients with complex care needs.

AGNES GRUDNIEWICZ
 PhD

Agnes Grudniewicz is an Assistant Professor at the Telfer School of Management, University of Ottawa, Canada. She holds a scientist appointment at the ‘Institut du savoir Montfort’ and is an affiliate investigator at the Bruyère Research Institute. Her research focuses on primary and community care, aiming to improve health care systems and services for patients with complex health and social needs.

PAULINE BOECKXSTAENS
 MD, PhD

Pauline Boeckxstaens is a family physician in the Community Health Centre ‘Botermarkt’ in Ghent, Belgium. She is also a professor at the Department of Public Health and Primary Care at Ghent University where she is building a research program on goal-oriented care. ​

JAMES MOLD 
 MD

Jim Mold is a retired family physician, geriatrician, researcher and leader who helped to reshape our thinking about health and health care. His is currently a George Lynn Cross Emeritus Professor of Family and Preventive Medicine at the University of Oklahoma, USA. His research includes a reconceptualization of health and health care around patient-relevant goals rather than clinician-directed strategies.

ALANA ARMAS
 BA, MSc

Alana Armas is a PhD Student in the Institute of Health Policy, Management and Evaluation at the University of Toronto, Canada. She is also a Research Coordinator at the Bridgepoint Collaboratory for Research and Innovation. Alana's research interests focus on integrating health and social services across healthcare and the community.
